Service Agreement
Top Tier Laboratory LLC offers drug testing and physical exam services for both DOT and Non-DOT purposes. By using our services, you agree that:
 

• All information you provide is accurate, complete, and up to date.

 

• You are authorized to undergo or order testing (as an individual or on behalf of an organization).

 

• You understand and agree to the service fees and procedures before scheduling or ordering.

 

• We reserve the right to refuse service to any individual or company that does not comply with our policies, is disruptive, or provides false information.

 

Payment Terms

 

• All payments are due at the time of service unless previously arranged under an employer billing agreement.

• We accept cash, credit/debit cards, and certain employer-issued purchase orders.​

 

• Online orders must be paid in full at the time of purchase to confirm booking.

 

• Refunds may be issued only in cases of service error or test cancellation prior to specimen collection.

 

• We do not offer refunds once a test has been conducted and/or submitted to the laboratory.

 

Appointment Policy

• We accept walk-ins based on availability, but appointments or online orders are encouraged for priority service.

 

• Clients must arrive on time. Late arrivals may result in rescheduling.

 

• No-show appointments without prior notice may be subject to a rescheduling fee.

Confidentiality & HIPAA Compliance
All test results and personal information are protected in accordance with HIPAA (Health Insurance Portability and Accountability Act). Results & Reporting

• Test results are typically available within 24–72 hours depending on the test type.

 

• DOT-related test results are reviewed by a certified Medical Review Officer (MRO).

 

• All results are delivered through secure electronic systems or by physical mail if requested.

 

• We are not responsible for delays caused by lab processing or incomplete paperwork.

Online Orders
Clients who place online orders through our website must:

• Provide accurate contact and identification details.

 

• Use the order confirmation to complete the testing process within 30 days (unless otherwise noted).

 

• Understand that misuse or fraudulent use of our ordering system is subject to legal action.

Businesses working with Top Tier Laboratory LLC must:
 

• Submit signed authorization or service agreements prior to employee testing.

Failure to pay or repeated rescheduling may result in service suspension.
